The real problem

Maintenance and Safety are almost never bought from the same side, and that's why they never talk to each other.

The CMMS lives in one system, the SSO in another, and the work permit in an Excel spreadsheet. The result: EPP is issued without knowing which activity it's for, the permit is signed without knowing if the equipment has overdue maintenance, and the accident is investigated without the asset record on hand. The same four gates open the work order and the work permit: the activity checklist, valid EPP or job safety analysis, the standard operating procedure (SOP) and the signed work permit/lockout (PTW/LOTO). If the asset doesn't have them up to date, neither the maintenance nor entry into the risk zone gets authorized.

Positioning
